Please submit your signed written request by post to:

Please include the following in your request:

  • Name (including names known by, or previous surnames)
  • Date of Birth
  • Address (including previous addresses)
  • Gender (including any change to this)
  • Telephone number and/or email
  • NHS number (if known)

There is a cost of £10 for print out only, or £50 for print out and Lloyd George note box.

Subject Access Request (SAR) Access to all medical records held, as set out by the Data Protection Act of 1998.

Summary Care Record (SCR) Brief medical information including medications, prescriptions, allergies.

A good practice time frame from receiving your request to acknowledging receipt is 21 days; from receipt to completion is 40 days. If you are not satisfied that we have met with good practice please do tell us. We are committed to improving our service to our patients, particularly our online services.
